It is a comfortable narrative because it assigns blame to inanimate infrastructure and administrative oversight. It lets everyone pretend that the system is fine if we just paint the yellow lines brighter.

The reality is far more uncomfortable. When a pedestrian is struck by a freight locomotive in a modern municipality, we are rarely looking at a simple accident.
